Registering on yet another website is a chore for sure, but we hope that the benefits will outweigh the inconvenience. A first-time sample submission is a three-step process, but subsequent sample submissions will be single-step.
Step One:
Complete the Client Registration Form. You will receive an email containing username and login information. With full access to the features of this web app, you will be able to download the sample submission form.
Step Two:
Download the Sample Submission Form. Once logged in, the download button will be visible below.
Step Three:
Fill out the sample submission form and email it to us (email address is provided in the sample submission form instructions page). Any issues or questions, Contact Us.
